Particular sub-speciality of Ophthalmology

Lifestyle diseases such as diabetees and hypertension are rising with an associated increase in vitreo-retinal problems leading to blindness. In Nepal, problems like age related macular degeneration, retinal detachment, dystrophy and retinal drug toxicity are also common.

The Vitreo-Retinal Setup at TIO has the necessary infrastructure, well-trained personnel and the capacity to handle a large number of patients suffering from retinal problems.

The following diagnostic procedures are available at TIO’s Vitreo-Retinal Clinic;

  1. Fundus Photography
  2. Diagnostic Ophthalmic Ultrasonography
  3. Fundus Fluorescein Angiography (FFA)
  4. Optical Coherence Tomography (OCT)
  5. Electrophysiological tests

The clinic also provides;

  1. Laser Therapy
  2. Intravitreal injections of steroids and anti-vascular endothelial growth factor
  3. State of the Art modern vitreo-retinal surgery for various VR conditions.

The establishment of a Pediatric Clinic had been a priority of TIO since its inception. In early 2012, the clinic was renovated as child-centered facilities and two dedicated pediatric ophthalmologists are actively involved in the clinic.

The pediatric ophthalmology clinic focuses on early diagnoses and treatment of different types of amblyopa-refractive, anisometropic, stimulation deprivation and strabismic amblyopia.

Surgical management of strabismus, pediatric cataract, penentrating and perforating injuries in children are all managed under general anesthesia.

Regular community outpatient and school screening services are conducted throughout Nepal.

TIO has a separate wing for glaucoma services to provide regular and extended care.

Patients with glaucoma need to be followed up and their condition monitored.

TIO recognized some years ago that the glaucoma unit needed to provide services beyond basic glaucoma care to patients, such as community screening, ophthalmic assistant training, patient education and public awareness programs.

As a result free education programs on glaucoma awareness are held throughout the year (usually once every two months).

A Glaucoma Support Group has also been established by TIO which helps the economically backward population with medicines and surgery. Glaucoma Awareness Week is held in March of each year.

The Orbit, Plastic and Lacrimal Clinic (OPAL) was established in 2004.

Maintining vision by protection of an eyeball and the proper function of the orbital adenexa, as well as cosmesis to functioning eye and oribit, is its main function.

For patients with disfiguration of eye lids and orbit, vision threatening ocular conditions and ocular and orbital tumours the OPAL service is available.

TIO has seen a significant increase in the number of patients being treated for orbital surgery.

Treatments offered at the OPAL Clinic include plastic and reconstructive surgery for conditions such as droopy eyelids (ptosis), inward turning eyelids (entropion), blocked tear ducts, and thyroid eye disease.

The OPAL Clinic also offers cosmetic and aesthetic treatments for various problems such as facial lines and wrinkles and surgery for baggy eyelids and drooping eyebrows.

Botulinum toxin injections for facial wrinkles and essential and hemifacial spasm is carried out at TIO regularly.

The Uveitis Clinic, which operates five days a week, has become very busy. TIO receives a large number of uveitis patients who have been referred from other eye centres in Nepal and from neighbouring India.

It is run by one uveitis specialist who deals with all sorts of presentations, namely, intraocular inflammation like anterior, intermediate, posterior and panuveitis, along with scleritis, sclerouveitis or keratouveitis.

Various types of endophthalmitis are also managed medically and surgically in co-operation with the retina unit. Complicated uveitic cataract is also managed at TIO.

infectious diseases like tubercuosis and leprosy are still prevalent in Nepal, and an increase in HIV infection and syphilis in the community has meant a spike in the number of uveitis cases presenting at the clinic.

Various supportive tools in the diagnosis and management of uveitis cases are available at TIO. These include the microbiology and pathology laboratory, ocular (anterior segment and fundus) photography, fundus fluorescein angiography (FFA), and optical coherence tomography (OCT).

Besides steroid therapy, various immunosuppressive therapies, laser therapy and intraocular anti-VEGF injections are also provided by the uveitis clinic.

Patients with neuro-ophthalmic problems are being identified in higher numbers due to the increase in facilities for diagnosis and treatment.

Cranial nerve palsy, optic neuritis, ischemic optic neuropathy, optic atrophy, myasthenia gravis and various kinds of brain tumours are just some of the frequently seen neuro-ophthalmic diseases.

A well equipped neuro-ophthalmology clinic is running with very dedicated neuro-ophthalmologists.

Low vision  is bilateral reduction in visual acuity which impairs ones work and cannot be adequately corrected with medicine, surgery, therapy, conventional spectacles and contact lenses. It mostly includes the loss of sharpness in acuity, loss of contrast sesitivity, reduced field of vision, increased sensitivity to light and distortion of image.

General Services

An OPD (outpatient department) is where modern ophthalmic care is given to the general public. From TIO's inception to-date interventions and modalities such as modern cataract, corneal transplant, glaucoma surgeries, vitreo-retinal surgeries, orbital surgeries, strabismus surgery and refractive surgery have been offered.

Laser Eye Surgery

Tilganga’s Refractive Surgery Unit opened in January 2012 and offers the latest in laser technology for the correction of corneal problems. It has been developed with TIO partners to become a leader in laser surgery in the developing world.

Eye Bank

The concept of the Eye Bank was supported by the International Federation of Eye and Tissue Banks (IFETB), before TIO was even built in 1994.
